Jiniaa Youth Services – Candidate Prescreen Questionnaire

Section 1: Availability & Flexibility

Section 2: Workplace History & Safety

The purpose of the following questions is to assess your ability to safely perform the inherent requirements of the position, as outlined in the job description provided to you, and to identify whether any reasonable workplace adjustments may be required. Information, including any health or medical details provided will be treated confidentially and used only for lawful employment, workplace safety, and reasonable adjustment purposes.

7. Do you have any current injury, illness, medical condition or restriction that may affect your ability to safely perform the inherent requirements of this role?
8. Are there any workplace adjustments or accommodations you may require to perform the role safely and effectively?
9. Are you currently taking any medication or receiving treatment that may affect your ability to safely perform the duties of a Youth Worker?
10. A Youth Worker may involve safety-sensitive duties including:
  • Operate a vehicle safely
  • Prolonged standing or walking
  • Climbing stairs or ladders
  • Repetitive movements
  • Manual handling or lifting
  • Running or responding quickly in emergency situations
  • Working extended shifts or overtime
